Patient Centered Medical Home RN- Primary Care (PT, 24 hrs)(full benefits)

The Patient Centered Medical Home Registered Nurse Works collaboratively with physicians, staff and other health care professionals to provide a medical home and care coordination across the health care continuum for all patients within the Primary Care Clinic. Is an integral member of the health care team who works to ensure that safety, best practice and high quality standards of care are maintained across the continuum. Responsible for coordinating a wide range of self-management support and disease registry activities for the clinic's entire patient population.

Graduate of an accredited school of nursing with an Associate's degree or Baccalaureate degree in nursing (BSN). BSN preferred. Candidates who do not have a BSN must be enrolled in a Baccalaureate program within 6 months of hire and must complete their BSN within 36 months of hire. Registration and current RN licensure in the state of MA in good standing required. Professional certification highly encouraged. CPR certification required; ACLS certification preferred.

A minimum of 2 years recent experience (within the last 5 years) in ambulatory office adult primary care is required. Ability to work independently and as a member of a multidisciplinary team. Demonstrates excellent interpersonal, verbal/written communication and delegation and problem-solving skills. Demonstrates strong analytical, data management and computer skills. Demonstrates strong organizational and time management skills.
